Third Party Authorisation

Here at Lincolnshire Housing Partnership (LHP) we take data protection seriously and work very hard to keep your personal information secure and safe. We also understand that sometimes you want someone else (a “third party”) to speak for you or act on your behalf. This could be, for example, a relative, a tenant representative, your local MP or Councillor, a solicitor, or a friend.

Under the UK General Data Protection Regulations (UK GDPR), as amended by the Data Protection Act 2018 (DPA), LHP needs your specific consent before we can release personal information which we hold about you to any third party, including those speaking or acting on your behalf. This is an important part of how we keep your personal data safe and secure.

We will also need to ask any third party a series of security questions when they contact us, so that we can verify their identity. We do this with all customers as it is an important part of our data security processes and in keeping your information safe.

All information provided by you and the third party will be kept securely on LHP’s information systems and disposed of securely when it is no longer needed. You can learn more about the LHP’s approach to data protection by viewing our Privacy Policy.

If you do want LHP to disclose your personal information to a third party and/or discuss your affairs with them, please read this guidance, complete and sign the form and return it to us:

To identify you, we will need your name, address and date of birth.

To prove your identity, we need to see a copy of your photo identification. Please indicate which ones you are supplying. If you don’t have any photo identification, please select the last box and upload a copy of your most recent utility bill, such as water, gas electric.

Please tell us who you want the information to be disclosed to. We will need their name and contact details as well as their relationship to you.

In the same way that we ask you for a password and questions for security purposes, we need the third party to use a password and answer security questions. These should be different from your own password and security questions. When the third party calls up, we will ask them to confirm your full name, full address and the password confirmed below. If they cannot remember the password, we will ask them for the answer to the security question below.

By ticking the below box, I hereby acknowledge my agreement and provide LHP with consent to disclose my personal data to the person(s) detailed at section 4 of this form. By submitting this form, I confirm I have read and understood the guidance and that the information I have submitted in this form is correct, to the best of my knowledge and belief. I confirm my understanding that LHP will only process and use the information I have submitted, in accordance with its Privacy Notice.

I understand that LHP reserve the right to refuse to act on my request if they are unable to verify my identity and/or my ability to provide consent. Finally, I understand that this consent is tenancy long, and will only be removed once I withdraw it myself. I acknowledge that I can change or withdraw my consent for this disclosure at any time and can do this by contacting LHP’s customer service centre via 0345 604 1472 or via email.
